What is a Breast Augmentation?

This procedure is frequently performed to restore volume to the breasts following weight loss and/or pregnancy.

Breast augmentation is a surgical procedure designed to enhance the size, contour, and symmetry of the breasts. The procedure consists of the surgical creation of a space either in front of the pectoralis muscles or behind them, inserting implants into that space in order to enlarge the size of the breast and improve contour. A wide variety of women who are looking to add and/or restore volume and contour can benefit from this customizable and highly individualized breast enhancing procedure.

Patients have several different options when deciding on their choice of an implant. Modern implants are filled with either saline at the time of surgery, or come pre-filled with silicone gel, and they come in a variety of shapes, sizes, contours, and surface textures. The doctor will go over each implant in detail during the consultation to assist you in deciding what your best option would be. There is also an optional way to enhance breast volume using a fat transfer technique, utilizing your own natural fat to give increased volume and shape.

Types of Implants

The two types of implants most commonly used are saline and silicone. Modern silicone implants are usually the preferred type, and they come in a variety of sizes, shapes, and surface textures. Depending on the patient’s anatomy and her particular goals, the choice of implant may differ. All implants are FDA approved; Clayton Plastic Surgery is a large provider of implants made by Allergan as well Sientra.

Saline

Saline implants are a silicone rubber shell that is filled with sterile salt water at the time of surgery after being inserted into the breast pocket. Because these implants are inserted before being filled, they require a smaller incision resulting in only a tiny scar. In the event of an implant rupture, the saline solution will be safely absorbed into the body, a process which is harmless. If that were to happen, it is not a medical emergency, but it is a cosmetic emergency! Saline implants are warrantied for life, so if they ever break or leak, the implant manufacturer will replace them free of charge. In fact, if one implant is ruptured, the manufacturer will replace BOTH implants. For a small additional fee up front (~$200) in the event of a ruptured implant, the company will also pay a large share of the cost of the surgery to replace them!

Silicone

Silicone implants are designed to feel more like natural tissue. These implants are filled with a safe silicone gel that provides a similar look and feel to natural breast tissue. This type of implant is pre-filled from the manufacturer. Gel implants are soft and natural, and they are extremely safe, strong and durable. The manufacturer is so confident of the integrity of gel implants, that if one ever fails for any reason, the manufacturer will replace BOTH implants, and if it occurs within 10 years of being put in, they will also cover a large portion of the cost of the surgery to replace them!

Gummy Bear

Over the years, the silicone gel in breast implants has been made more and more cohesive. That means that the gel is made less and less runny, so that almost any modern implant, even if cut in half, will not leak or deflate. They look something like a chewy gummy bear candy inside, hence the nickname “gummy bear” implants. They are the latest in form stable implant technology. The main benefit of gummy bear implants is that they hold their shape. Additionally, modern cohesive implants cause less rippling or wrinkling of the skin.

Additional Implant Characteristics

Implants come in a variety of sizes; Dr. Clayton offers implants that range from 150cc to 800cc. They also come in two different shapes: round or anatomical. Round implants provide equal projection at the top and bottom of the breast, while anatomical implants have a sloped tear-drop shape more like the shape of a natural breast. While “anatomical” implants may seem like the best way to go, it turns out that even round implants become “anatomical” once inside the body, and in fact, most plastic surgeons can’t even tell the difference in “after” photographs of patients with round and those with anatomically shaped implants. Anatomic implants are also more expensive. For most women, round implants work great!

As stated above, each implant brings its own advantages and disadvantages. This is why Dr. Clayton is here to assist and guide you throughout the decision-making process. Breast Augmentation vs. Breast Fat Transfer

While breast implants continue to be the easiest and simplest way to enhance breast volume, some women today are choosing to use their own body fat instead. Fat transfer for breast augmentation has gained popularity because it only requires that you have some extra fat somewhere (most of us have plenty of extra!). The fat can be harvested from the “problem area” with a liposuction technique and transferred as a living tissue into the breasts. Not all of the fat will survive this transfer process, but typically, around 50 – 70% will survive and become a living part of the patient’s new breasts. The result will be soft and natural and will last indefinitely.

One potential problem with fat transfer for breast enhancement is that since not all the fat survives the transfer process, there is a small degree of uncertainty about exactly how large the new breasts will ultimately be. It can take a few months to determine the final breast size. There is also a limitation to how much fat that can safely be transferred in a single operation. Occasionally, a second surgery may be needed if the patient wants very full breasts by fat transfer. With breast implants, the final size is known beforehand, since the size of the implant is predetermined in consultation with Dr. Clayton.

Neither one of these procedures is necessarily better than the other. There are many factors that go into making the decision of which one to use. That’s why Dr. Clayton’s experience can be invaluable in helping you choose the operation which is best for you.

Candidates

Women who are looking into breast augmentation should be in good general health with a healthy diet and exercise program. Ideally, the patient should also be a non-smoker. Women who are considering this procedure should have realistic expectations about the look they want to achieve. It may not always be possible for a woman who wears a small A cup to increase to a DD in a single surgery. Such changes are possible, but more extensive surgery may be required.

Ideal candidates for a breast augmentation should not be currently pregnant, or breastfeeding. After the surgery, it is possible to have and nurse more children.

Once the consultation is complete, if you are ready, you can schedule a date for the procedure and receive all the instructions that must be followed leading up to the procedure. Procedure

Before beginning the procedure, Dr. Clayton will first review with you the goals of the surgery. He prefers the inframammary fold incisions but can perform any incision technique. Once this presurgical interview has been completed, our certified anesthesiologist will administer the general anesthetic in order to guarantee your comfort throughout the entire procedure.

Dr. Clayton then re-measures and marks the important anatomical landmarks to ensure symmetry and will make the initial incisions. He then makes the space in which the implants will be placed. These pockets can be made over or under the muscle, depending on what was discussed during the consultation. Dr. Clayton generally prefers to place them in front of the muscle but will place them behind if the patient has inadequate natural breast tissue. The chosen implants, either saline, or silicone, will then be placed into the breast pockets.

Once Dr. Clayton has placed the implants, he will recheck the result to make sure he has obtained the desired shape. When he is satisfied with the results, the incisions will be closed with tiny dissolving sutures, and you will then be taken to the recovery area.

Recovery

Once in the recovery room, you will be monitored by our nurses for an hour or two before being released to go home.

Dr. Clayton rinses out the implant pocket with a long-acting anesthetic so that there is very little – if any – pain in recovery! During this time, you will receive recovery instructions and your appointment date to come back in for the first post-op visit. Suture removal is not necessary since the sutures dissolve. Patients are advised by Dr. Clayton to walk briefly the night of surgery in order to help improve circulation and reduce the risk of blood clots. The bra that is provided must be worn constantly throughout the recovery period. This will help with any bruising or swelling.

Follow-up appointments will be scheduled in order for Dr. Clayton to properly keep track of your recovery.

You should refrain from showering and daily activities until your first post op visit and should wait 5-6 days before engaging in normal activities. Then, at 1-2 weeks, you may perform more strenuous activities. You may experience slight swelling and tenderness throughout the recovery process, but this is completely normal.

Results

You will notice an immediate increase in the size of the breasts, but the full results will be seen a couple months after the procedure. Swelling will subside and breast contours actually improve for a few months. Only then will the final result be fully apparent. The breasts will be more elegant, clothes will fit better, and your confidence is enhanced.
